Diagnostics and therapy of auditory processing disorders in children

The general rules of diagnosis and treatment of Auditory Processing
Disorders (APD) in children are presented. APD are
diagnosed often in children starting school, when the demands
for hearing abilities are elevated. Children with APD have difficulties
with understanding speech in noise and in the classes
with high reverberation. The diagnosis of APD should be conducted
by the audiologist in consultation with speech language
pathologist and psychologist.
The clinically most useful groups of higher auditory function and
related auditory tests include: auditory attention and auditory
memory and the efficacy of transfer of information between
the brain hemispheres – dichotic digit test (DDT); temporal
resolution and auditory pattern recognition – frequency pattern
test (FPT) and gaps in noise test (GIN); auditory performance
decrements with competing acoustic signals – speech in noise
test.
Electrophysiological test like cABR and late cortical and cognitive
auditory potentials are useful in difficult to diagnose cases. Therapy
of APD is based on auditory training designed to improve
such auditory skills like sound localization, understanding speech
in noise, sound discrimination and auditory attention and memory.
Development of suitable compensatory techniques is an
important element of the therapy. Good results can be achieved
by improving signal-to-noise ratio through application of hearing
aids dedicated specifically for APD children.
