Subjective and objective acoustic phenomena in patient with parafunction of stomatognathic system – a case report

Introduction. Hyperactivity of jaw muscles and structural
anomalies in stomatognathic system (SS) may lead to ear
symptoms.
Aim. Report a case of a young woman suffering from
temporomandibular disorders (myalgia, temporomandibular
joint skipping) accompanied by objective and subjective
otologiccomplaints.
Material and methods. A 23-year woman treated in the
Departamentof Oral Rehabilitation, Medical University in
Poznań for parafunction of SS was diagnosed in the Department
of Audiology and Phoniatrics, University of Medical Sciences
for tinnitus.
Results. The audiological examination was normal. The
stomatological diagnostic procedure revealed prevalence of
unconscious parafunctions. Psychologic examination revealed
an emotional conflict. The applied therapy, including audiological
and stomatological treatment and psychotherapy, resulted in a
considerable improvement – subjective tinnitus disappeared,
the problems with temporomandibular joint were alleviated.
However, the objective tinnitus generated by middle ear
muscle myoclonus proved to be resistant to the therapy and
still continues.
Conclusions. Floating tinnitus without pathology in the
hearing organ coexisting with otalgia and fullness in the ear may
suggest problem in SS. The psychogenic factor may be of major
significance in the etiology of tinnitus and parafunction of the
stomatognatic system. Interdisciplinary approach is necessary.
