The influence of stapes surgery on the bone conduction threshold in a long term follow-up

Introduction. The level of bone conduction in otosclerosis
is affected by several factors: cochlear otosclerosis, Carhart’s
effect and the possible injury to the inner ear caused by stapes
surgery or post-surgery complications. Therefore, assessment
of changes in bone conduction in otosclerosis is difficult. It is
not clear whether stapes surgery accelerates deterioration of
bone conduction threshold, or possibly protects against damage
to the inner ear caused by otosclerosis.
Aim. To assess the influence of stapes surgery on changes
of bone conduction in otosclerosis in a long term follow-up
period.
Material and methods. The changes of bone conduction
threshold in the operated and non-operated ear were evaluated
in the group of 48 patients (selected from among a total of 600
people operated for otosclerosis) with bilateral otosclerosis
after unilateral stapes surgery in a follow-up period of 5-19
years.
Results. The mean bone conduction threshold in the operated
ear improved in the early period after the operation almost by
9 dB, and was app. 19 dB compared to nearly 28 dB before
surgery. In the long period of follow-up, the mean bone
conduction threshold in the operated ear deteriorated by
8 dB. Similar results were noted in the nonoperated ear. The
differences between the values in operated and nonoperated
ears were not statistically significant.
Conclusions. Uneventful stapes surgery improves bone
conduction threshold in the early period following the surgery.
However, it does not affect the changes of bone conduction
threshold in otosclerosis in the long follow-up period after
surgery.
