Effects of voice therapy in a patient with paralytic dysphonia – a case study

The paper presents a description of voice rehabilitation in
a patient who was diagnosed six months earlier with idiopathic
paralysis of the left half of the larynx. Prolonged untreated
paralytic dysphonia, with sustained pathological compensation,
had an adverse impact on the psychosocial functioning of the
patient. A comprehensive diagnostic method was applied,
including subjective self-assessment by Voice Handicap
Index (VHI) and scale of vocal tract discomfort (VTD),
GRBAS perceptual evaluation by a physician, followed by
videolaryngostroboscopy, acoustic voice analysis and maximum
phonation time measurement (MCF). After 3 months of intensive
voice rehabilitation, the whole test procedure was repeated. A
significant improvement in subjective symptoms as measured by
VTD scale – both in frequency subscale (23 vs. 5) and severity
subscale (22 vs. 5) was achieved. A similar improvement
was found in self- assessment by VHI for the outcome of
the total score (62 vs. 16) and in perceptual evaluation by
GRBAS (G1R0B0A0S0 vs. G3R0B3A3S2). Desirable effects
of the rehabilitation were also, manifested by improved
aerodynamic objective parameter (17 seconds longer MCF after
rehabilitation), acoustic parameters (shimmer, APQ, NHR), and
the parameters evaluated in videostroboscopy. Presented case
study indicates to the usefulness a comprehensive diagnosis of
paralytic dysphonia and complex assessment of the effectivness
of voice therapy.
