Allergy to latex – Aetiology, symptoms and treatment

Paper presents current knowledge on latex allergy regarding its history, characteristics of major allergens, patients at risk, clinical manifestations, available diagnostic methods, recommended treatment and prevention principles. In general population frequency of latex allergy ranges between 0.66-18.3%, latex anaphylaxis morbidity is 0.4%, while in perioperative settings it is up to 20%. Patient groups at risk are mainly children with ventricular-peritoneal shunt due to spina bifida, health care workers, mainly in operation theatre, children either with congenital malformations requiring many surgical interventions or those after spinal cord injury. Examples of clinical manifestations of latex allergy include irritant eczema, allergic contact dermatitis and symptoms of type I immediate reaction. The latex-fruit syndrome due to cross-reactivity between latex and some plant (banana, kiwi, avocado) allergens is a special type of clinical manifestation. Diagnostics of latex allergy is based on clinical history, skin prick test results, in vitro test with detection of latex specific IgE to whole latex, to latex major allergenes (ImmunoCAP) or certain epitopes (ISAC) as well as provocation tests. Medical interventions in treatment of clinical symptoms of latex allergy do not differ from those applied in the course of allergy to other allergens. There are preliminary attempts to introduce sublingual immunotherapy into the treatment armamentarium of IgE-mediated symptoms of latex allergy. Implementation of the principles of both primary and secondary latex allergy prevention has been shown to be effective and is a method of choice.
