Abnormalities of the immune response in primary immunodeficiency disorders may be associated with either deficiency or excess of immunoglobulin E production. Pathogenesis, clinical manifestations and questions concerning diagnostic procedures of the immune system are discussed in connection with essential hyper-immunoglobulin E syndromes (hyper-IgE syndrome, Wiskott-Aldrich syndrome, Omenn syndrome and Comel-Netherton syndrome). Particular attention is paid to regulatory mechanisms of IgE synthesis, complexity of the immune phenomena, and genotype - phenotype relationshp. Chances for application of the increasing knowledge about mechanisms governing IgE production in the clinical practice are also discussed.
